Who we are
We specialise in managing frailty, cognitive problems (when a person has trouble remembering, learning new things, concentrating, or making decisions that affect their everyday life) and people with multiple medical problems. Our team are also experts in managing gait disorders (imbalance, shuffling, frequent falls, staggering, and freezing), frails and fracture prevention.
How we help
We provide full geriatric assessment for our patients in A&E and acute medical unit (AMU).
RACOP (Rapid Access Clinic for the Older Person) provides rapid clinic assessment for any patient with frailty or complex needs, irrespective of age and accept referrals from GPs, community services, acute services and A&E.
We also support with inpatient reviews for fraility issues for all specialities of the hospital for complex fraility problems, which can’t be managed by specialities’ own disciplinary team.
Each of our consultants has their own specialist interest; Parkinson’s disease clinics, community work in bed based intermediate care, virtual support to community teams such as CRRS (community rapid response service) and home visits. Our team also have strong links with the community falls and osteoporosis service, our GP colleagues and local voluntary organisations.
