Learning disabilities liaison team
Our learning disabilities liaison team provide care and treatment that meets your needs. This includes making any reasonable adjustments.
The team help people by:
- offering longer appointment times
- supporting them in understanding their treatment plan
- preparing them for hospital visits or stays
- supporting hospital staff to communicate well.
We also encourage you to bring any tools that you use such as communication boards with you to hospital.
My Care Passport
We use a form called ‘This is me – my care passport’ to help you or your carer to tell us important information about you and your needs. We ask you to keep the form updated each year.
